Cashless Claim Procedure

Contact hospital insurance helpdesk with a copy of patient’s medical Records / E-health card, along with valid Govt. ID proof.

Hospital Insurance help desk will share the Pre-authorisation form along with supportive documents to our TPA seeking approval.

In case any information is incomplete/missing; TPA may contact hospital for the same.

Claims approval will be done as per Policy terms & conditions.

At the time of Discharge, discharge summary and other related documents are submitted by the hospital to our TPA for final approval.

Post discharge, we will settle the claim payment directly with the hospital.

Reimbursement Claim Procedure

Intimate your claim within 24 hrs of hospitalisation to Care@royalsundaram.in

If you get treated at a non-network hospital, you will need to pay the treatment costs upfront and collect all treatment and expense-related documents.

Submit all the Documents along with the Claim form to us directly within 30 days from the date of discharge.

We will process your claim as per policy terms and conditions.

Once approved, we will reimburse the claim amount to you.

Here’s an exhaustive list of documents you may need to make a claim. You may need few or all of them based on your situation.

Required Documents

  • Duly filled and signed claim form - Click here to download Claim Form
  • Discharge summary
  • Original final hospital bill with item wise detailed break-up
  • Original paid receipt against the final hospital bill
  • For Accidental cases - MLC/FIR copy (if applicable)
  • All Investigation reports (like Lab Reports/ X-Ray/MRI/CT scan etc)
  • All pharmacy bills supported by doctor's prescription
  • Implant sticker / invoice, if used (eg: lens details in cataract case, stent details in angioplasty)
  • Past Medical Records /Treatment Papers wherever applicable.
  • NEFT / KYC Details of the Proposer (Cancelled Cheque / Copy of Pan card etc.)
  • Other relevant documents, if any.
